Dude I did a whole level with only the Katanna, but never got the achievement for only using melee
I think it means all 5
first one I got was for killing a spitter before she spits
beat the whole story on normal going to try something harder but with friends.
I found out what it means and basically it means only use melee in one campaign and only melee, which means no bombs either
